ER crowding may not slow heart attack care - Health Nutrition and ...

NEW YORK (Reuters Health) ? An overcrowded ER does not seem to delay patients in getting an emergency procedure to stop a heart attack in progress ? at least at one U.S. hospital, a new study finds.

Over the past couple of decades, the number of emergency rooms in the U.S. has dropped by about one-quarter, even as the number of patients visiting them has climbed. That often equals ER overcrowding and research suggests it?s taking a toll.

A recent study of California ERs, for instance, found that elderly heart attack patients had a higher death rate when their nearest hospital was on a high level of ?diversion? ? meaning the ER was so crowded it was turning ambulances away.

And some research, though not all, has suggested that when heart attack patients arrive at a crowded ER, their care may be slowed down ? whether they require an invasive procedure or blood-clot-dissolving drugs.

For the new study, reported in the Annals of Emergency Medicine, researchers at Advocate Christ Medical Center in Oak Lawn, Illinois, evaluated the treatment given to 210 heart attack patients at their suburban hospital.

All of the patients received emergency angioplasty, wherein a balloon-tipped catheter is threaded into the arteries to clear the blockages causing the heart attack.

Experts say that when a patient needs emergency angioplasty, it should happen within 90 minutes of their arrival at the hospital.

In this study, patients waited an average of 65 minutes. And the level of ER crowding did not appear to affect that wait time.

On average, the ER was filled to 127 percent of its capacity, but across the study period conditions ranged from 28 percent full to 214 percent.

Dr. Ben Harris, who led the study, called the findings ?somewhat reassuring.? But he added that U.S. ERs are still operating beyond capacity and it remains a general problem.

?I don?t think there?s any doubt that overcrowding is having an adverse effect overall,? agreed Dr. Erik B. Kulstad, who also worked on the study.

However, those critical patients who need an emergency heart procedure are given priority.

According to Kulstad, ER crowding may not have had an effect in this study because the hospital has a plan in place to speed the right patients to angioplasty.

?When an ultra-critical patient arrives requiring treatment and intervention, that is carefully measured?resources to care for the patient are brought in from not just the emergency department alone, but from throughout the hospital,? Kulstad explained.

The flip side to that, he noted, is that it may draw staff and resources away from other patients? care.

The main limitation of the study is that it focused on one hospital. It?s not clear if the same results would be seen at other centers with different levels of staffing, for example, or those in settings other than the suburbs.

But Kulstad said they suspect the findings could be similar in other U.S. hospitals, since all centers now have to publicly report their heart attack treatment outcomes.

As a result, he said, they?re putting high priority on cutting the wait time to treatment.

The one factor that did affect patients? wait time in this study was whether they got to the hospital during ?business hours? or off-hours.

Patients who arrived on a weekday between 6 a.m. and 6 p.m. ? when the angioplasty team was on-site ? started treatment in 55 minutes, on average. If they arrived on a weekend or at night, when the angioplasty team was on-call, they waited 76 minutes.

Of course, people can?t decide when a heart attack will strike, or whether the ER will be crowded once they arrive.

The main thing they can do, Kulstad said, is to call 911 if they think they are having symptoms of a heart attack ? which can include pain or pressure in the chest; pain in the arms, neck, jaw or stomach; breathing difficulty; light-headedness and nausea.

In the future, though, Kulstad noted, people may be able to get information on crowding in their local ERs.

There are plans to publicly report hospitals? ER-crowding rates on the government?s Hospital Compare website (http://www.hospitalcompare.hhs.gov) starting in 2014.

Public reporting is also considered a good motivator for hospitals to find ways to improve.

Children and fitness should go hand in glove for every family. I?m here to say that it will also make a family healthier overall and that is a win win situation. Not only that but I believe it can make a family much happier.

There is no reason for us to dread fitness especially when there are countless numbers of fun activities the whole family can do together. This not only promotes health and happiness, but it also brings families together which is something we need more of today.

Children And Fitness Ideas

All of the kids exercises I?ve come up with are not only kid friendly and easy to do, but they are also designed to have the whole family participate. Sounds great right!

I know without a doubt that there are parents who are searching for extra time to get some exercise in themselves and this is a perfect way to get that accomplished. Yes, you can run on your treadmill or ride your exercise bike when the kids go to school or bed, but wouldn?t you much rather save time and get it done with the rest of the family?

Take a hike ? Hiking through the woods is great exercise for the whole family. If you live in a city, be willing to drive to a national park so your kids can get the experience of a real hike. Otherwise, most cities have a place nearby to allow people to hike and picnic.

Not only will you get good exercise, but I can personally testify that hiking in the woods or park is quite relaxing and peaceful, which is something we all need from time to time.

Go bowling ? Believe it or not, this fun family sport does help your kids get some exercise. Lifting, carrying and rolling a bowling ball will work their arm and leg muscles.

Play two full games and make sure you?re going at a nice pace because you don?t want to be there for hours. The better pace you have, the better exercise you?ll get.

Your children learn by watching you, after all, so a whole-family approach is key. It doesn?t matter if your child is one, five or ten. They pick things up quick and I know that firsthand as I?m a parent of two.

You have to watch what you say, watch how you react, watch how you treat other people and you have to watch how you treat exercise and nutrition.

When we are good examples for our kids then it will help everyone in your home develop better habits and that?s what we?re after. We want healthy lives for ourselves and our kids.

I know from personal experience that children and fitness equals a healthier and happier family and you can achieve the same thing.

It's only a year until the London Olympics, but maybe there's still time to introduce a new event. Many Londoners are putting in some intensive training in Synchronised Looting and Throwing Things at People, but none of this is particularly impressive on an athletic level.

If they want a real challenge they should try the Gibbon Leap. These apes can leap over 10 metres from tree to tree. But the strange thing is that they do it without any of the specialised adaptations that other leaping animals have evolved.

Gambolling gibbons

White-handed gibbons live in tropical forests, spending almost all their time in the trees. They are one of the few animals that brachiate: they swing themselves along the undersides of branches using only their arms. That's until they reach the edge of the tree they're in, at which point they jump across to the next.

Animals that do a lot of leaping tend to have all sorts of subtle adjustments to their bodies that make them better at it. For instance, leaping animals such as frogs have unusually long hind legs, which means the force of the jump is transmitted over a greater distance.

Gibbons have none of that, yet they have been seen jumping over 10 metres horizontally. What's more, they generally jump slightly further than they need to, presumably to be sure they don't fall. Anthony Channon of the Royal Veterinary College in Hatfield, UK, and colleagues decided to find out what their secret is.

Ape leap set

Last year Channon found that they jump in four distinct ways, depending on the situation. For short distances, they would walk along a branch and then jump from one foot, like an extended step. Sometimes they brought both feet together before the jump.

They also performed squat leaps: they sat with both legs bent at the knees, then extended them while also raising their arms, hurling themselves through the air and landing on their feet. Jumping this way costs a lot of energy, but sitting still may give them a chance to take a close look at the tree they're jumping to, ensuring it is safe.

And for the biggest jumps, they started with one leg in front of the other and pushed themselves off with the rear one. This time they caught the destination branch with their hands, and often went straight into a bout of brachiation.

Channon has now taken high-definition videos of captive gibbons leaping, and analysed them in detail. The gibbons covered distances up to 5.2?metres, and took off at speeds up to 8.3?metres per second.

To achieve that speed, they had to use 34.5 joules of energy for every kilogram of body mass. He says that is a record for a single movement by any animal. Humans, by comparison, manage just 6.3 joules per kilogram. Clearly, the gibbons can marshal a lot of energy.

The trick, according to Channon, is the way the apes swing their upper bodies and arms. This means the force of the leap is being transmitted along much of the length of their bodies, amplifying its effect.

This is rather similar to a trick used by ancient Greek athletes when performing the long jump. They would carry heavy weights called halteres in each hand, each one weighing up to 9 kilograms. Before jumping they swung them back and forth, then forwards as they took off and backwards as they landed.

It sounds counter-intuitive, because adding extra weight ought to shorten the jump. But in fact the halteres moved the athlete's centre of mass forwards and up at take-off, allowing them to jump further. A 2002 study showed that doing this would have extended a 3-metre jump by 17 centimetres at least.

Given apes' talent for tool use, it probably won't be long before gibbons start clutching rocks.
